Due to Illinois Youth Soccer Association Policy we must now keep an "IYSA Emergency Medical Release and Liability Waiver" on file for EACH player registered to play with Homer Soccer Club. This applies to both Recreational and Travel soccer players.

Re: Travel players - If you are currently registered with and playing for a Homer Hawks FC team, you probably have a "Medical Release Form" on file. If any information has changed (address, phone, insurance information, etc) we ask you please complete a new form and give it to your team administrator or coach.
